Following Information Ready:
- Students Name
- Graduation Year or Last Year Attended
- Date of Birth
- Social Security Number (optional)
- Name and Address Where Records Are To Be Sent
- Fee, $2.00 For Each Copy Of Official School Transcript (cash or money order
only) If Records Are To Be Sent Electronically to a In-State University, There
Is No Fee.
